This surveillance and monitoring instrument is employed by professionals or trained paraprofessionals to determine if a child's development is within the normal range. It is a useful instrument for EPSDT and identifying children at risk. The results are not diagnostic. It is to be used with children birth to six years of age.

Forms come in pads of 100, with an age scale based on The American Academy of Pediatrics recommended health maintenance visits and a behavior rating scale.
