Prescreening Developmental Questionnaire ( PDQ II )
The PDQ is a parent answered questionnaire requiring 1-2 minutes of office personnel time to instruct parents, and less than 10 minutes for the parent to complete. Health professionals find the instrument useful and a time saver, often using it as a first step in assessing a child's development. Providers also find PDQs encourage parent discussion of child development with them.
The forms come in pads of 100, available in English and Spanish.
